Guidelines for submission of lacking documents:
1. Documents for dropping at the REGISTRAR box in Navotaas Institute Annex II:

b. Original copy of Senior High School Card (Grade 12)
c. Original copy of Certificate of Good Moral
d. Photocopy of PSA/ Birth Certificate (if copy is from Local Civil Registry, submit the a notarized undertaking) https://docs.google.com/…/1oE16bFPuwAxdjZzvZJcG…/edit…
e. Photocopy of Official Receipt for payment of Entrance Examination
f. Photocopy of 2021 cedula of student, for minor, cedula of parent
g. 2 passport size picture with nametag
LAST DAY OF SUBMISSION : OCTOBER 4, 2021
2. Email npc.registrar@gmail.com for appointment schedule for the submission of the following documents:
a. Certification that Form 137 was not issued to any school
b. Affidavit of discrepancy if the name of student in the High School card is not the same in the PSA/ NSO Birth Certificate
c. Photocopy of Navotas City Comelec stub or voter’s certification if student is 18 y/o and above
3. Observe health protocols. Wear a face mask and face shield. Bring your own ballpen and alcohol.
